Apostille Requirements for a New York Affidavit

Privately prepared affidavits generally follow New York’s standard authentication process for notarized documents.

Completed Affidavit

The affidavit should clearly state the declaration being made and identify the individual making that statement.

Proper Notarization

The affidavit must be properly executed before a commissioned New York notary public using the appropriate notarial act.

County Clerk Authentication

The notary’s signature must then be certified by the County Clerk where the notary is qualified before submission to the New York Department of State.
